Possibly the book "Operating and Evaluating School Library Media Programs,
a Handbook for Adminisrators and Librarians," (Neal Schuman, 98) could
prove helpful.  Appendix C has a form, but the entire book may be of
interest if you are in the evaluation mode.  Authors Yesner and Jay.

Go to your local library and either check out or interlibrary loan Connecting
Young Adults and Libraries by Patrick Jones.  There is a survey in it.  I
used that survey as a starting point and edited it to my needs.
