The Teachers & Writers Handbook of Poetic Forms, edited by Ron Padgett.  New
York: Teachers & Writers Collaborative, 1987.    The Handbookcontains
seventy-four entries by nineteen writers.  An example is given of each.

I don't have a book suggestion for you but I did use this web site last year
when doing a poetry unit in our computer lab. I don't know if you have

Hope you can find it useful,
How to Write, Recite, and Delight in all Kinds of Poetry by Joy N. Hulme and
Donna W. Guthrie. Brookfield, Conn. The Millbrook Press, 1996.
This book contains a chapter on the forms of a poem including free verse,
pattern poetry, wordplay, rap, and syllable poems. It also includes a
chapter on figures of speech.
I checked and it is still in print, which in and of itself is a
recommendation!

Good Morning Collegues.

My sixth grade teachers are looking for a book or books that gives the
definition of each kind of poems and also shows an example.  For instance, a
hiaku, limerick, etc.  Thanks so much, I've looked through all of my
catalogs, nothing!  TIA
