Student referral form

I have been asked by the attendance officer of our District to locate
sample referral forms that teachers might use to refer students that they
feel to be "at-risk".  This would be  an initial step to determination of
eligibility for programs that might help these students.

We are searching the literature but perhaps some school district has
practical tips to offer.
