Target--->>Evaluating a school library program

I have been asked to help prepare for my school's evaluation by members
of the  Council of Administrators of School's for the Deaf.  One of the
questions was to list the weakness of our program.  I'm sure we have
some! But because it is really hard for us to find anything (us being
those who work in the library/lrc...) I would like to pose some questions
to the staff we serve.

Does anyone have a pointer to a developed questionnaire?  What have
others used to solicit this sort of feedback from 'patrons'?
