Portfolio Help

As our school heads into the world of portfolios, our prinicipal would like
some examples of "best practices".  After reading all the literature and
attending workshops, we would like to know what schools are including in
portfolios.
(1) What is commonly included in your school for a student portfolio (9-12)?
(2) How do you determine what is included in a student portfolio?
(3) How do you store your portfolios?
