Department Chair Conference Forms

Our English Department Chair asked me to put this out to all the
middle and high school "netters."  She is looking for a form that can
be used when she conferences with teachers in her department.  Do any
of your department chairs have a form or forms that might serve this
purpose?  If you do if you will either mail or FAX me a copy, I would
greatly appreciate it!   Thanks!
