REF,SEC:FORMAT FOR COURSE SYLLABUS

The Director of Accountability in our system is requiring that each faculty
member at our high school submits a course syllabus for each course.  Does
anyone have a "generic" syllabus format that you might be willing to share
with us?  Our fax number is 615-967-6945.  Thank you for any help that you
might provide.  My ERIC searches and searches in our professional
collection have been unsuccessful.  I can find specific subject area
syllabi, but not a general format that all could use.
