Equipment Circulation

        Does anyone have a special form for teachers to sign that enables
them to check out equipment--laptop computers and/or computers, printers,
cd players?  If you would send me a copy or inform me as to what
information you request, are they responsible for any damage, theft, loss
etc.?  If so, how did you word that in your form?  Any help would be
appreciated for we have new equipment and the principal wants teachers to
use them whenever they know they will need them.  It may be for personal
or school business.
