REf:Footprints *poem*?

     We are doing a poetry unit involving a bibliography.  One student wants to
use *Footprints* which he has a home on a wall plaque.  He says the author is]
unknown. Th gist of the *poem* is that there are two sets of footprints when
his lord is walking with the author.  When there is only one set of footprints
it is because his lord is carrying him.
    I am vaguely familiar with this piece, but I am not sure it is a poem.
Could anyone tell me where I could find it written in a book?  Otherwise should
the citation read:
     "Footprints."  Plaque.
