Summer reading programs

A while ago I asked all of you if you have required summer reading
programs and what methods of evaluation do you use.  Here is a summary of
the ten people who responded.
    Grade levels & population
      2 HS (all students)
      1 HS (English and history classes only)
      2 HS (AP or Honors English)
      2 MS (all students)
      1 Elem (suggested reading list - not mandatory - if they read 10 or
more books they receive a free paperback from the library
      1 Private (5-12) - 5 book reports over the summer, 3 must be from a
list.

     Evaluation formats (these were mentioned - some gave serveral options)
        4 Test
        3 Journals
        2 Evaluation method is up to teacher
        2 Book report
        2 Oral presentation
        1 Discussion in groups
        1 Pick a quote from each chapter
