I am using a wonderful little book:

THE ST. MARTIN'S POCKET GUIDE TO LIBRARY RESEARCH AND DOCUMENTING SOURCES.
(1991) ISBN 0-312-05688-5

It includes examples of documenting sources in:

Modern Language Assn (MLA) format
American Psychological Assn (APA) format
Council of Biology Editors (CBE) format

Generally, we are expecting Middle School students to use APA for Science
Fair project research and MLA for Humanities research.
