Formative IEP

Would you please check with your Special Education Department and ask if
they know anything about the new fomative IEP form.  We have heard rumors
about this form, but have not seen one.  Please help if possible.

Reply as soon as possible.  If you have a copy of the form please send as an
attachment.  We work from an IBM Microsoft Works program.
