SEC: Surveys on service and climate sought

Hi all,

        Has anyone administered a survey for students and faculty on the services 
you
offer in your school's media center?  How about a survey regarding the climate or a
"welcoming" atmosphere students and faculty perceive in the media center?
        I have been asked to conduct such a survey, and I need some help in knowing 
the
type of questions (and the actual questions) I should have and shouldn't have to 
avoid
having anyone take "pot shots" at us.  (Yes, we're vulnerable and sensitive about 
this!)
