GEN:Video permission forms

Hi all,
    Our district will be videotaping students and showing these tapes to
audiences both inside and outside of the school community. Does anyone have
special forms to request permission from the parents to allow their children
to be in such a video?  Is there a difference if the student is identified?
If you wouldn't mind sharing I would appreciate it. I plan to search the Web
tomorrow at school, but unscheduled time is at a premium. TIA to a wonderful
and helpful group.
