Student's pictures on Web pages

Dear LM_Netters,

We are constructing a web page that will include student's pictures.
We have decided to have a permission form filled out by the student's
parents. Does anyone already have this policy and have a permission
form in place. We have a AUP policy that all students parent's have
to sign but this will be a different form. Thought you all might be
able to save me some time constructing one.
