State school media award criteria

Good Morning!
  The Delaware School Library Media Assn (DSLMA) has an annual award for
an outstanding librarian and his/her program.  We are looking to upgrade
our criteria and would like to see the criteria on which other states base
their awards. We'd  like to shift our emphasis to an exemplary program.
  I would truly appreciate it if anyone could email or snail mail me a
sample of their award application or questions used in determining their
state award.
