barcodes

I have some questions for those of you with automated systems.  If a book comes
back without the barcode, do you assign a new number or make a new barcode
using the old number?  If you have duplicate titles, how do you (quickly)
determine the correct barcode #.  If you have duplicate barcodes placed in the
book, where do you recommend they be placed?  Thanks.
