GEN: teacher surveys

I have to write a survey for teachers for a course I am taking. This
survey is supposed to cover such topics as curriculum consultation with
the LMS, what types of reading guidance are provided, types of
information provided, and ways teachers use the library and the LMS.
Ideas? Any good surveys?
