TARGET: How to Write Report Card Comments

I have a teacher who wants a book that will give some examples for writing
report card comments.  I have some kind of vague memory that just such a
book exists but can't seem to put my hands on a title.  We have some
teachers here for whom English is a second language and a "how-to" book
like this would be very helpful.  Do any of you know if it exists?  I need
title and order information.  TIA  :-)
