Evaluating videos

Hello, friends,
Does anyone have a form and guidelines for evaluating videos?  At our
sytem office we have thejob of locating, buying and lending to shcools a
very large video collection.  I would like a way to quickly evaluate
videos recived on apreview basis and would like to have a form and
criteria so tha my staff can help.  Thanks in advance.  Barbara Wall,
