mark read scanners for scoring

We're looking for recommendations of mark read scanners to be used in
scoring tests, etc.

Specifically,
     Names and models of any you'd recommmend
     Names and contact info of companies producing or selling these
     Any info on cost of scanner and of supplies (cards)?
