Gen: Tech. survey suggestions

Our school is going through it's accreditation this year and since our
last check, technology did not have a committee all it's own.  A survey is
sent out to alumni and community people to evaluate the different areas
included in the accreditation evaluation.  What I am asking is imput, in
the way of questions, that can be put on this survey that deals with
technology in a school that can then be rated on a 1-5 point system.
Example:  Computers are easily accessible for student/classroom use each
period of the day.
Strongly Agree      Agree       Sometimes    Disagree   Strongly Disagree
     5                4             3            2             1
The technology questions can deal with all areas in the curriculum.
