Closed circuit TV

Does anyone out there use a form for teachers requesting to have a video
played over their closed circuit distribution system?  I need one &
wonder if you have them write down the objective & subject of the
lesson.  Our principal is kind of lax in allowing teachers to bring in
their videos that they own to use in their classroom, but I'd like to
cover the bases when they want me to show it.
