Libr. Media Spec. Evaluations

Friends!  Hello and Good morning,
One of our high school librarians is being evaluated by her department
chair -English Depart- and  he has freely admitted he doesn't know how.
Chance for her to educate and provide him with a good evaluation form
and for us  to spread it throughout the county.  Does anyone have a good
evaluation for a library media specialist and library media center?  The
librarian found part of one from Kentucky but has not been able to
retrieve the rest from the Kentucky School Media web page .  any help
would be appreciated and if you can fax me copies, thanks.  Fax is 315
