Student Aides

I plan to start using student aides soon (grades 5-8) and was wondering if
anyone has an application form they'd be willing to share with me.  I'd like
to make this experience similar to applying for a job, so I'd like students
to fill out an application and be interviewed.  You could attach it to email,
fax (517-485-5892), or send by snail mail.
