I've been asked to post a hit of the responses I got concerning
reconsideration policies, so here they are:

As I see you are asking two separate questions.  The first question is
about language and terms usually used in selection policies and requests
for reconsideration of materials used in the curriculum.  The
reconsideration process usually involves a written request to have
material removed from the library or curriculum.  In the case of a
student who objects to reading a supplemental book why wouldn't they
just be able to chose another book from the list that they like better?
The book would remain on the list for the rest of the students.  For two

Our district policy manual includes a form for parents and citizens to
lodge written complaints against materials.  Written complaints go to
school committee, with appeal to district committee.  Materials remain
in school until final action is taken on complaint.

The policy also states, "However, the school may observe the request of
a parent that his/her own child not be assigned a specific book."  Our
school has a policy of assigning alternates at parent request.  Happens
about once a year in a school of 320 students.
