Thank you to all 38 people who responded to me on this question. It
doesn't seem that there is any universally accepted way to do this! But, it
settled the difference of opinion between the teacher and I. Quite a few
people wrote asking me to post a HIT, so here it is. I tallied up the
responses and rather than post them all I'll just give the numbers.

My original question: When there is more than one city listed for a
publisher, should the first city
listed be chosen for citation purposes, or should the city to which one is >
geographically closest be chosen?

Answers:

Geographically Closest - 12
First City Listed - 20
First U.S. City Listed - 6
First U.S. City Listed; if no U.S. City then Geographically Closest - 1

So, what do we do??? Does anyone have a source they can reference to
support their answer? I had one person say that they scanned the new MLA
Handbook and didn't find anything. So, who knows?
Thanks again to everyone who wrote!
