New media specialist

I am a brand new media specialist beginning mid-August.  I have found
lots of materials in my media center that I can't believe are widely
circulated among the staff.  Obviously, I don't wish to deselect
anything until I know who is using what and how often.  Also, just to
bring me up to speed on the feelings and attitudes about the media
center by the staff, I would like to send out a questionnaire with the
newsletter I am publishing.

My question is:  Has anyone ever polled his/her staff on their opinions
and uses of the media center and can I get a copy or should I reinvent
the wheel?  Obviously, it will have to be tailored to my media center
but I would like to get some feel for what I should include.
