Secondary: Survey

I am still hoping that someone has a survey to give to students to get
some idea of what books they would like to see us order this year.  Our
circulation increased last year, but I am still not happy with student
pleasure reading.  I want to be able to get what they want to read.  I
wonder if any of you have such a survey already prepared.  Or maybe we
could make one up together right here!  Who would like to start with a
good question? Such as, What was the last good book you read?
