GEN: LMS Evaluation

Denise Irwin from Andover High School/Butler County Community College has
faxed me a copy of a Library Media Specialist Evaluation Form that she and
the administration worked out several years ago.  I've noticed that several
list members have requested such a form in the past and I have been unable
to find and archived posts directly relating to websites or other sources.

I will fax a copy to those that need such a form.  Please send me your fax
number if you want a copy.  If I get too many requests I will scan the
document and send it as an image attachment.
