Evaluation of Public Librarians

Hi-I received a strage request from a teacher who serves on a public library
board. She needs to locate an evaluation tool for the public librarian.
They are evidently in the process of putting together job descriptions, etc.
and she is having a diffcult time locating an example to help in the writing
of one for her library.  Does anyone have an idea of where to look for this
