Video use forms--are there any out there?

I posted a request for video use forms on Friday, and have received
about 5 replies already--ALL asking for a HIT!  So...if you have one for
your school, district or county, please send it on to me, so I can post
for everyone else!

This is especially for copyrighted videos that are brought in to use in
class from video stores or personal collections.  I would like to take
ideas from these and create one that requires the teacher to put down
where they got it, how they are using it in a lesson plan, and even
standards that will be met.
