target: reconsideration form

LM_Netters:

I am trying to update the form I have for reconsideraton of library
material.  I am board chair of a small public library and we are
reviewing policy.  One of the policy items that came up was the form
that someone might have to fill out if they object to material in the
library.  We have a form, but would be interested in other choices.
