Hit: Form for flexible scheduling

A while back, I posted a target for a flexible scheduling form that included
library skills to be covered.  I received many request for a hit on this
topic.  A few responses (a special thank you to Judy Smith and Maureen Irwin
for sending me their samples) helped me create my own form for each grade
level.  The forms include all the necessary information for planning a
integrated lesson with the classroom teacher, and a listing of skills for
Library as well as Technology.   If you are interested in seeing these forms
for our K-8 school done in Microsoft Word, please e-mail me. (Of course it
is based on the needs of my students so feel free to modify it as needed.)
