Our high school is in the process of moving into a new building that will have a media retrieval system in the media center. This type of system is new to all of us and we (librarians in the district) have been asked by our high school principal to create a video clearance/permission form that staff members must submit to the Media Center when they are wishing to use video materials other than from the Media Center collection to show in their classrooms. Is this a normal procedure? Does anyone have any such type of form in use currently at your school? Are you willing to share the contents of the form so we can compare various ones that are currently in use around the U.S. and then design one that meets our needs? If you are using a media retrieval/distribution system in your school, would you be willing to share some of the pros and cons of having one? Any words of advice?
