GEN: LMS evaluation form

I have noticed the thread concerning a good evaluation form for us, as we
are often evaluated by standards that don't truly reflect our mission and
job. Although it is not mandated to be used in our evaluation in Kentucky,
we are making great strides in its implementation. In fact, I was evaluated
by this standard for the first time this year. You may see the form on the
