TARGET: Permission to Checkout

A committee is currently reconsidering a book in the elementary library (A
View from the Cherry Tree by Willo Davis Roberts, because of profanity in
the text).  My principal has heard that one school has handled a similar
controversy by requiring the parents to sign a form before allowing the
book to be checked out.  Do any of you have a similar policy? Would you be
able to fax or snail mail a copy of the form?  Any comments, pro/con?  TIA!
