reconsideration of materials

My district is looking to update the form which we use when a parent
challenges library materials.  Our current form only deals with print
materials.  Does anyone have a form which also includes non-print or do you
use a separate form for print and non-print materials.  If anyone is
willing to share a model of what your district uses I would appreciate it.
