Release Form online?

Hi...during workshops we do at our Education Service Center
in San Antonio, Texas, educators create web pages and a
variety of other products. Their work is posted on our web
site (http://www.esc20.k12.tx.us/etprojects/). We've
recently asked them to submit a release form for each
project they create and is posted on the web site. However,
the release forms are starting to stack up (we train
hundreds with multiple projects).

Is is possible to have an <a
href="http://www.esc20.k12.tx.us/etprojects/training/edtech_
release.html"> electronic release form </a> that workshop
participants fill out online, submit, and then that is kept
online? This make it searchable. Here's the information the
form would collect:

1) Full Name
2) Email address
3) Project Title
4) Grade Level
5) Type of Project
6) Other Contact Information as needed
The text of the form would be similar to the paper release
form.

What do you folks think? Would it work?
