Gen: Curriculum Review Cycle for Library/Media Program

Hello,
We are in the process of evaluating our Library/Media programs in our
district.  This process resembles the same review cycle as the other
curriculum areas in our district. We look at our mission statement, goals,
materials, scope and sequence, graduation standards, staff development, etc.

We are looking for surveys that other districts or schools may have used to
help in the data gathering section of this review process.  We will be
surveying students, staff, parents, and community members.  If you have
something that sounds like it may help us could you please send your samples
via e-mail or fax.  We are looking for any type of questionnaires you may
have used to evaluate your libraries, collections,  curriculum, technology,
and much more.  We would greatly appreciate it.  Thank you so very much!
