Elem: Flex Access Tracking

Have any of you developed a check sheet for tracking teacher/class
participation in flex access planning and media center use?  With 40
teachers, I think I need to keep track of who I need to work with for equity
in our media program.  Please contact me personally if you have such an
instrument that you would be willing to share.
