Here is one of the things that I do after teaching a few lessons and providing a reference page. I have a series of forms (usually 4 to 6 fit on a page) that I keep in a partitioned box--one for book sources, internet sources, encyclopedia sources etc. On that form is all the information that is required for the bibliography in the correct order and with the correct punctuation. Students in the upper grades are free to use them any time they do research. They staple the sheets to their research and then use them to fill in the bibliography. This often alleviates forgotten information and then having to find the source all over again. For the 5's, I require a little less information.
