LM_NET: Library Media Networking
Good Morning,
I need help in finding a blank template of a survey form I can use
to evaluate library service. I looked in the archives and a few messages
looked promising, but no actual attachment or form was there. What I am
trying to do is compose a survey of about 10 questions that offer a range or
scale of 1-5 of patron satisfaction with library resources, service,
accessibility, etc. I am trying to learn more about the library's areas of
strength as well as weakness. Room on the form will also be provided for
additional comments. I hate to re-invent the wheel if one is out there,
thanks in advance for your help. I'd be happy to post a hit if there's
