Nutrition and fitness materials for an ESL high school student

Help!  We are looking for resources, both online and print, to use with an =
ESL Spanish-speaking, low-reading level (both languages) 11 th grader who =
has been assigned a nutrition and fitness project in place of physical =
education classes.  We would like to find Spanish and English materials on =
a fifth-grade reading level.  Any help will be appreciated!  Many thanks =
in advance.
