Elem: AV inventory

 I was wondering how some of you handle your AV inventory.  Since my library
does not have enough space for storage of AV equipment during the summer,
teachers keep equipment.  I am in the process of developing a sheet to hand
out to teachers so they can note which items they have and which they do
not.  Does anyone have such and sheet and is willing to share it with me?
