I received 3 replies and several requests for a hit, so here it is: (Thanks
again)

Dear Kris:  We used to have one of these forms that we put in
teachers' mailboxes each week.  On it we wrote the teacher's name and
regular lab time.  We wanted it to be simple and quick for teachers to
fill out, so we had a place for them to circle yes or no if they were
coming to the lab this week, and then we listed all the programs we
had in the lab and had them check which ones they would be using.

This worked well for awhile, until administration started wanting to
see this information, to check up on which teachers were regularly
coming to the lab, what they were doing, etc.  I did not feel that I
should be involved in what amounted to evaluating other staff members,
so we quietly "forgot" to do this.  It's kind of a shame, since it was
very helpful to the library/lab staff.
