TARGET: Need title of adult book

I'm looking for the title of a book, written about 15 years ago,
by a prolific novelist.  It's about two married couples who meet
in an academic setting, remaining true friends for 30 years or so.
The wife of one couple is physically limited by a degenerative
disease. As the book opens, this pair has travelled to the luxurious
home of the other couple, because the wife has a terminal illness.
As the book ends the latter wife accomplishes her goal of dying the
way she wishes to die, despite her husband's inability to support her
in this.  Does this sound familiar to anybody -- I hope?
