GEN: Examples of questionnaires on library usage by teaching staff

Does anyone out there have any examples of questionnaires on library usage
(and opinion of) by teaching staff.
I am the librarian at a large British comprehensive with sixth form (pupil's
ages are from 11 to 18). Any information would be wonderful.
