At-risk student survey

Our curriculum coordinator has requested help in finding an attitudinal
survey about school and education to be used with 9th grade at-risk
students - a sort of before and after survey, I think.  She has gathered
anecdotal information based on interviews and needs this written instrument
to more scientifically measure changes after their participation in a summer
program. Much of the anecdotal information focuses on student perceptions of
why they have failed classes.
