Evaluation Form

Dear all -
Before I spend hours on the 'net, I will ask the collective wisdom of the group
- do you have a form that your teachers use to evaluate administrators, 
specifically,
principals? If so, would you be willing to send me a copy as an attachment,
or via snail-mail?
